Definition

Oral medication calculation is determining how many tablets/capsules or how many milliliters of liquid to administer so the patient receives the exact ordered dose, based on the strength available. Oral (PO) drugs come in two families: solid forms (tablets, scored tablets, capsules) and liquid forms (solutions, suspensions, elixirs).

Purpose (Why This Matters for the Nurse)

Accurate oral calculation ensures the patient gets a therapeutic — not toxic or sub-therapeutic — dose. Just as important, the nurse must know the form rules: some tablets can be split, most cannot be crushed, and liquids must be measured with the right device. A correct number on paper is still unsafe if the form is handled wrong.

The Core Formula

Amount to give = Desired dose (D) Dose on hand (H) × Quantity (Q)

D / H × Q = X

D = desired/ordered dose
H = strength you have on hand
Q = the form the “have” comes in (1 tablet, or the mL the liquid strength is stated per)

Dimensional analysis (Lesson 2) reaches the same answer — use whichever you trust. Convert to the same unit first if the order and label differ.

Solid Forms — Tablets and Capsules

Example 1 (same units): Order 500 mg; available 250 mg tablets.

(500 mg ÷ 250 mg) × 1 tablet = 2 tablets

Example 2 (convert first): Order 1 g; available 500 mg tablets.

Convert 1 g = 1000 mg → (1000 mg ÷ 500 mg) × 1 tablet = 2 tablets

Example 3 (scored tablet): Order 0.125 mg; available 0.25 mg scored tablets.

(0.125 mg ÷ 0.25 mg) × 1 tablet = ½ (0.5) tablet — allowed only because the tablet is scored.

Solid-form rules: only scored tablets may be split, and generally only in half. Do not break or crush enteric-coated or extended-release (ER, XL, SR, CR) tablets or open certain capsules — it destroys the delivery system and can cause an overdose. Round tablet answers to a whole number or, for scored tablets, to the nearest half.

Liquid Forms — Solutions & Suspensions

Liquid strength is stated as an amount per volume (e.g., 125 mg per 5 mL). Q is that volume.

Example 4: Order 250 mg; available 125 mg / 5 mL.

(250 mg ÷ 125 mg) × 5 mL = 10 mL

Example 5 (dimensional analysis, with conversion): Order 0.5 g; available 250 mg / 5 mL.

0.5 g × 1000 mg 1 g × 5 mL 250 mg = 10 mL

Measuring liquids: use an oral syringe, calibrated cup, or dropper — never a kitchen teaspoon. Shake suspensions well before pouring. Read volume at the bottom of the meniscus at eye level.

Rounding Rules for Oral Doses

Unscored tablets/capsules: give a whole number only — never split.
Scored tablets: may round to the nearest half.
Oral liquids: commonly rounded to the tenth of a mL (follow facility/order policy).
Use a leading zero, never a trailing zero (0.5 mL, not .5 or 1.0).
An answer that requires many tablets or an extreme volume is a red flag — recheck.

Safe Oral Administration Points

Position the patient upright and confirm the ability to swallow (aspiration risk); stay until swallowed.
Oral route is contraindicated if the patient is NPO, vomiting, unconscious, or has impaired swallowing.
Do not crush enteric-coated/extended-release forms; check a reliable “do not crush” reference.
For enteral tubes, use liquid forms when possible, give each drug separately, and flush before, between, and after.
Give irritating drugs with food when indicated; give others on an empty stomach per instructions (e.g., levothyroxine).

Apply the rights and three checks; verify high-alert oral drugs (e.g., warfarin) with a second nurse.
Sanity-check: does the number of tablets/volume make sense for one dose?
Measure liquids with a proper device; shake suspensions.
Respect form rules — scored-only splitting, no crushing ER/EC.

NCLEX Clinical Judgment Points (CJMM)

Recognize cues — the order, the available strength, the form, and swallowing ability.
Analyze cues — are the units matched? Is the form splittable/crushable?
Prioritize hypotheses — safety first; airway before administration.
Generate solutions — convert, calculate (D/H×Q or dimensional analysis), choose the measuring device.
Take action — administer correctly, upright, and confirm the swallow.
Evaluate outcomes — check the amount is reasonable and the patient tolerated it.

Reasoning frame: a safe oral dose is usually a small, sensible number. If the math says “4 tablets” or “40 mL,” pause and re-examine units and decimals.
