Why This Matters

Prevalence: HIV affects over a million people in the U.S. and millions more worldwide—it is a chronic, manageable disease today rather than the rapidly fatal illness it once was, but only if patients stay on therapy. Nurses in every setting—primary care, obstetrics, med-surg, mental health, long-term care—will care for people living with HIV.

Patient safety: untreated or poorly controlled HIV progressively destroys CD4+ T-helper cells, the "generals" that coordinate the immune response. As counts fall, the body loses the ability to fight organisms it would normally handle easily, leading to opportunistic infections (OIs) and AIDS-defining cancers. ART adherence is the single biggest driver of a normal lifespan.

Analogy — an army that kills its own generals

Think of the immune system as an army with CD4+ T-helper cells acting as generals who direct two other units: the B-cell artillery (antibodies) and the cytotoxic T-cell infantry (cell-killing). HIV doesn't attack the enemy directly—it infiltrates and kills the generals themselves. Early on, there are still enough generals to run the war. But as HIV steadily kills more and more CD4+ cells, fewer generals are left to give orders, and the army becomes disorganized. Eventually there aren't enough generals left (CD4+ < 200) to defend against enemies the body used to defeat easily—opportunistic organisms invade unchecked. That's AIDS: not one infection, but the loss of the ability to fight any of them.

What It Looks Like

In acute infection, the viral load spikes and CD4+ dips (flu-like illness), then the immune system partially controls it during clinical latency—CD4+ slowly declines while the person may feel well for years, especially without treatment being needed to know they're infected. Without ART, viral load eventually climbs and CD4+ falls below 200, defining AIDS, where opportunistic infections take hold. ART flattens this curve at any stage—viral load drops toward undetectable and CD4+ recovers.

Pathophysiology

Normal physiology: CD4+ T-helper cells recognize antigens and direct the immune response—activating B-cells to make antibodies and cytotoxic T-cells to kill infected cells. This coordination is what allows the body to fight viruses, fungi, and certain bacteria and cancers. ↓ What goes wrong: HIV binds to the CD4 receptor (with a co-receptor, CCR5 or CXCR4) on the T-helper cell surface, fuses with it, and inserts its RNA. Using reverse transcriptase, the virus converts its RNA into DNA, which integrase inserts into the host's own genome. The hijacked cell now manufactures new virus, and protease cleaves viral proteins into their mature, infectious form. Each cycle **kills the host CD4+ cell** and releases thousands of new virions to infect more cells. ↓ How symptoms appear: during acute infection (2-4 weeks post-exposure), the massive viral replication causes a flu-like/mononucleosis-like illness (fever, sore throat, rash, lymphadenopathy)—this is the immune system's first, dramatic battle with the virus. As CD4+ counts slowly fall over months to years (clinical latency), the person often feels well because enough CD4+ cells remain to hold most infections at bay. ↓ How it progresses: once CD4+ falls below key thresholds (roughly <500, then <200), the immune system can no longer suppress organisms it used to control effortlessly. Opportunistic infections (Pneumocystis pneumonia, candidiasis, toxoplasmosis, CMV, TB, MAC) and AIDS-defining cancers (Kaposi sarcoma, non-Hodgkin lymphoma, invasive cervical cancer) emerge—this is AIDS. Without ART, death typically results from an opportunistic infection or malignancy, not HIV itself. NCLEX Clinical Pearl — why CD4+ count IS the immune system's "battery level" Think of CD4+ count as a battery percentage for the immune system. Above 500, the system runs fairly normally. Between 200–500, the person is more vulnerable to common infections and some early OIs (thrush, shingles, TB). Below 200, the battery is critically low—PCP, toxoplasmosis, cryptococcal meningitis and other severe OIs become likely, which is exactly why prophylactic medications are started at specific CD4+ thresholds (see Medications).

HIV is transmitted via blood, semen, vaginal/rectal secretions, and breast milk—not through casual contact, saliva alone, tears, sweat, or insect bites.

Why acute HIV is so often missed Acute retroviral syndrome looks just like a viral flu or mono—fever, sore throat, aches, lymphadenopathy. Because the symptoms are nonspecific and resolve on their own, patients and providers often don't test for HIV at this point. Yet this is also when the viral load is at its highest and the person is most infectious. The lesson: ask about recent exposures/risk factors whenever a patient presents with an unexplained mono-like illness.

Nursing Assessment

Subjective: ask about known HIV status, date of last negative test, sexual history, injection drug use, needlestick/occupational exposures, recent flu-like illness, weight loss, chronic diarrhea, night sweats, and recurrent infections (thrush, shingles). Approach with a nonjudgmental, confidential, trauma-informed manner—stigma is one of the biggest barriers to testing and care.

Objective: inspect skin (rashes, Kaposi lesions, shingles), oral cavity (thrush, hairy leukoplakia), lymph nodes (generalized lymphadenopathy), weight trend, respiratory status (cough, dyspnea, hypoxia suggesting PCP), and neurologic status (headache, confusion, vision changes suggesting CNS opportunistic disease).

NCLEX pearl — CD4+ and viral load move in opposite directions On effective ART, viral load should trend DOWN (toward undetectable) while CD4+ should trend UP (immune recovery). If you see viral load rising while CD4+ is falling or flat, think nonadherence or drug resistance—assess adherence barriers before assuming treatment failure.

Safety — the abacavir hypersensitivity black box Abacavir carries a Black Box Warning for a potentially fatal hypersensitivity reaction (fever, rash, GI symptoms, malaise) most common in patients who are HLA-B*5701 positive. Never test the drug by rechallenging after a suspected reaction—repeat exposure can cause a rapid, severe, and sometimes fatal reaction. Test HLA-B*5701 status before the first dose. NCLEX testing points ART is always given as combination therapy (never monotherapy). Adherence is everything—teach patients that skipping doses, not the drugs themselves, is what causes resistance. U=U (Undetectable = Untransmittable): a person with a sustained undetectable viral load cannot sexually transmit HIV—an important, evidence-based teaching point. Know the abacavir/HLA-B*5701 pairing and the CD4+ < 200 → PCP prophylaxis pairing cold.

Why adherence is the intervention Unlike many acute illnesses where the nurse's hands-on actions cure the problem, in HIV the nurse's biggest impact is often keeping the patient engaged in daily treatment for a lifetime. Every missed dose gives the virus a chance to replicate and mutate. That's why adherence counseling, stigma-free support, and problem-solving barriers to taking medication are just as much "nursing interventions" as any physical task.

Patient Education

Disease process: explain that HIV attacks the immune system's CD4+ T-helper cells but that modern ART controls the virus effectively, allowing most people to live a normal lifespan when taken consistently. Medication adherence: emphasize taking ART every day, at the same time, without skipping doses—stress that missed doses (not the drug) cause resistance, and to call the provider before stopping any medication due to side effects. Lifestyle modifications: practice safer sex (condoms, PrEP for partners), avoid sharing needles, maintain routine vaccinations (inactivated vaccines are safe; live vaccines require CD4+ evaluation), eat a balanced diet, and avoid raw/undercooked foods when CD4+ is low. Home care / follow-up: attend regular CD4+/viral load monitoring visits, take prescribed OI prophylaxis as directed, and keep all follow-up appointments even when feeling well. Partner & family: discuss partner notification and testing, and reassure that HIV is not spread through casual contact (hugging, sharing utensils, toilets). U=U: teach that a sustained undetectable viral load prevents sexual transmission, which can meaningfully reduce fear and stigma. Warning signs / when to seek emergency care: teach the patient to seek immediate care for new shortness of breath, high fever, severe headache with stiff neck or confusion, vision changes, or a suspected drug hypersensitivity reaction (rash with fever after starting a new ART drug).

Clinical Judgment (NCSBN Model)

Recognize cues: a 34-year-old with known HIV, last CD4+ count 180 cells/mm³, reports 3 weeks of progressive dry cough, low-grade fever, and shortness of breath with minimal exertion. He admits he has missed several ART doses over the past month due to cost concerns.
Analyze cues: CD4+ < 200 + subacute respiratory symptoms + missed ART doses = high suspicion for Pneumocystis pneumonia (PCP), likely worsened by nonadherence allowing viral replication and further immune decline.
Prioritize hypotheses: PCP (or another opportunistic pulmonary infection) is the priority to rule out and treat; ART nonadherence is the underlying driver that must also be addressed.
Generate solutions: obtain SpO₂/ABG, chest imaging, and sputum/BAL studies as ordered; anticipate TMP-SMX therapy; assess and address the adherence barrier (cost—connect to a case manager/assistance program) rather than simply repeating "take your medication."
