Definition

Team member responsibilities are the specific functions each level of the nursing team is legally and educationally prepared to perform: the RN (full nursing process, judgment, unstable patients), the LPN/LVN (directed care of stable patients under RN/provider supervision), and the UAP/AP — unlicensed assistive personnel, including nursing assistants, CNAs, patient care technicians — who perform routine, judgment-free care delegated or assigned by a nurse (NCSBN, 2019; ANA & NCSBN, 2019).

Analogy: Picture a commercial kitchen. The RN is the executive chef — designs the menu (plan of care), tastes and adjusts every dish (assessment/evaluation), and answers for everything that leaves the kitchen. The LPN is the line cook — highly skilled, executes recipes precisely, notices when a sauce breaks and tells the chef, but doesn't rewrite the menu. The UAP is the prep cook — essential, trained, does defined repetitive tasks exactly as instructed, and reports what they see ("we're out of onions") without deciding what it means for the menu.

Purpose

Every NCLEX assignment/delegation question is really asking: do you know which team member this patient and this task belong to? Matching the right role to the right work protects patients from care that exceeds a caregiver's preparation — the core patient-safety rationale behind licensure itself.

Foundational Framework (in place of Pathophysiology)

Responsibilities are distributed along two axes: judgment required and patient stability. As either rises, the work moves up the ladder toward the RN. The nursing process is the dividing line: the RN performs all five steps; the LPN participates in implementation and data collection; the UAP performs selected implementation tasks only, with no independent role in any step. Accountability is layered — each member answers for their own performance, the delegating/assigning nurse answers for the decision and supervision (NCSBN, 2019).

Normal Findings — What Each Role Correctly Does

RN responsibilities:

Initial, comprehensive, and admission assessments; assessment of all unstable or changing patients
Nursing diagnoses; developing, initiating, and modifying the plan of care
Initial and discharge teaching; evaluating learning and outcomes
IV push medications, initiating blood products, titrating drips
Delegation to and supervision of LPNs and UAP; evaluating all delegated care
Clinical judgment: triage, interpreting data, deciding what findings mean

LPN/LVN responsibilities:

Focused assessments and ongoing data collection on stable patients
Medications by PO, SQ, IM, topical, and enteral routes (IV varies by state)
Sterile procedures: wound care, urinary catheterization, suctioning, established tracheostomy care, ostomy care, enteral feedings
Reinforcing teaching the RN initiated; documenting and reporting findings
Monitoring stable patients with chronic, predictable conditions
Directing UAP in many states

UAP responsibilities:

Activities of daily living: bathing, hygiene, grooming, dressing, toileting
Vital signs, height/weight, intake and output, capillary blood glucose (where validated), on stable patients
Ambulation, transferring, turning and repositioning, range-of-motion (established/routine)
Feeding patients without swallowing precautions; passing meal trays; fresh water
Collecting non-sterile specimens (urine, stool, sputum); bed making; 1:1 safety observation
CPR (if certified) and immediately reporting all observations to the nurse — reporting, not interpreting

Abnormal Findings — Role Violations

UAP performing any assessment, medication administration (outside state-certified med-aide programs), sterile procedures, teaching, or care of unstable patients; UAP evaluating anything (e.g., "UAP reports the lung sounds are clear" — auscultation is assessment)
LPN performing admission assessments, initial teaching, IV push/blood initiation, care planning, or taking the unstable patient
RN delegating any nursing-process step, or "delegating" without supervision
Anyone performing tasks their competence was never validated for, regardless of role

Causes (of role confusion)

Short staffing pushing tasks downhill; job titles mistaken for competence; long-term-care and clinic settings where custom has blurred lines; float and agency staff unfamiliar with facility role definitions; nurses unclear on their own state NPA.

Signs and Symptoms (of a responsibilities breakdown)

A UAP charting "patient tolerated ambulation well" (that's an evaluation); an LPN completing admission paperwork including the assessment; abnormal glucose values sitting unreported because the UAP didn't know the reporting threshold; an RN unaware of what her team members did all shift.

Nursing Assessment — The Assignment-Making Self-check

For each patient/task pair ask: How stable is this patient? How much judgment does this task require for this patient today? Whose validated competence covers it? What supervision can I actually provide this shift? Assign the most stable, most predictable work down the ladder; keep new, unstable, and judgment-heavy work at the top.

Nursing Interventions — Building a Safe Assignment

Apply the Five Rights of Delegation to every handoff. Give UAP task-specific instructions with numeric reporting parameters ("report SBP <100 or >160, temp >100.4°F, glucose <70 or >200 — immediately, not at end of shift"). Pair the LPN with stable patients whose needs are procedure-based. Schedule your own follow-up checkpoints — assignment isn't complete until the RN has evaluated results and patient outcomes.

Patient Education

Teaching belongs to the RN; reinforcement to the LPN; the UAP may remind patients of instructions already given ("remember the nurse said to call before getting up") but never introduces, explains, or evaluates health information.

Safety Considerations

Every team member — including UAP — has the duty to refuse tasks beyond their training, and the nurse must honor that refusal. The most dangerous phrase in team nursing is "can you just…" attached to a task that quietly crosses a role boundary. Two-way communication (SBAR for reporting up, closed-loop instructions going down) is the safety net that catches most role errors before they reach the patient.

NCLEX Clinical Judgment Points (CJMM)

Recognize cues: map every stem to two variables — patient stability and judgment required. Stable + routine → UAP; stable + skilled/procedural → LPN; unstable, new, or judgment-laden → RN.
Analyze cues: identify the hidden destabilizer (first dose, post-op day 0, new symptom) that moves a patient up the ladder.
Prioritize: the RN personally takes whichever patient requires assessment now.
Take action: assign with parameters, not vague vigilance.
Evaluate: the RN closes every loop — reviewing UAP data, LPN reports, and patient outcomes personally.

Common NCLEX Traps

Giving the UAP a task that's routine in form but not in circumstance (vitals during a transfusion, feeding a new stroke patient — swallowing evaluation isn't done); letting the UAP's report substitute for nursing assessment ("UAP states patient is fine" is never an assessment); overrestricting the LPN (sterile procedures ARE theirs) while underrestricting them (teaching is NOT); options where the RN keeps busywork and hands judgment away; forgetting that "obtain a glucose" is delegable but "decide what to do about it" never is.

Five NCLEX Clinical Pearls

Sort every option by two questions: Is the patient stable? and Does the task require judgment? Two nos = UAP; skilled task + stable = LPN; anything else = RN.
The UAP collects data (numbers, observations); only nurses attach meaning. "UAP reports glucose of 52" is correct; "UAP treats hypoglycemia" is never.
Feeding is UAP-appropriate ONLY when swallowing is not in question — new stroke, sedation, or dysphagia history pulls the tray back to the nurse.
When the RN, LPN, and UAP could all technically do a task, NCLEX wants it assigned to the lowest-level team member who can safely perform it — that's efficient, correct delegation.
The RN's non-negotiables spell a pattern: every FIRST (first assessment, first teaching, first dose response), every UNSTABLE, every EVALUATE.

Five Common Student Mistakes

Assigning by task difficulty instead of stability + judgment — then giving the UAP "easy" vitals on a transfusion patient.
Accepting UAP observations as assessment findings instead of triggering a nurse's own assessment.
Forgetting UAP can do capillary glucose checks (where validated) but cannot act on the result.
Splitting hairs wrong on teaching: LPN reinforces (correct), UAP "explains the low-salt diet" (violation — reminding ≠ explaining).
Assuming the RN should personally do all hands-on care to "be safe" — failing to delegate is also a management error NCLEX penalizes.

Five Application Scenarios

Team assignment: RN, LPN, and UAP share six patients. New admission with GI bleed → RN. Stable diabetic needing dressing change and PO meds → LPN. Stable hip-replacement day 3 needing ambulation and vitals → UAP. The logic: judgment → RN, skilled procedure/stable → LPN, routine/stable → UAP.
UAP report: The UAP tells the RN, "Room 12's blood pressure is 84/50, but he says he feels fine." Correct RN action: assess the patient personally now — UAP data triggers nursing assessment; it never replaces it.
Feeding assignment: The UAP is assigned to feed a patient admitted yesterday with a new stroke. Error: until a swallowing evaluation clears the patient, feeding involves aspiration-risk judgment — the nurse takes it back.
Glucose check: A validated UAP obtains a bedside glucose of 58 on a stable patient. Correct sequence: UAP reports immediately per parameters → nurse assesses and treats per protocol → RN evaluates response. The UAP obtaining the value was correct; anything further wasn't hers.
LPN vs. UAP boundary: A stable patient needs a urinary catheter inserted and an unoccupied bed made. LPN takes the catheter (sterile procedure), UAP the bed. An option assigning the catheter to UAP violates role; an option making the RN do both when she has an unstable patient elsewhere fails prioritization.
