Definition

Communication is the exchange of information, thoughts, and feelings between people through verbal and nonverbal means. In nursing, the goal is therapeutic communication — a purposeful, patient-centered, goal-directed interaction that builds trust, gathers information, and supports the patient's wellbeing. It differs from ordinary social conversation: it always centers on the patient's needs, not the nurse's (ANA; Potter & Perry).

Communication also includes professional communication — the accurate, structured transfer of information among the healthcare team, most often using SBAR (Situation, Background, Assessment, Recommendation) (Joint Commission).

Analogy: Think of therapeutic communication like being a skilled interviewer, not a talk-show host. A talk-show host makes it about themselves and fills every silence; a good interviewer asks open questions, listens far more than they speak, and lets silence do work. In nursing, the patient is the star of the conversation — the nurse's job is to draw them out, not to take the stage.

Why it matters: Communication failure is the root cause of the majority of sentinel events (serious, preventable patient harm) reported to the Joint Commission — most often during handoffs and provider notifications. Good communication isn't "soft skills"; it is a core patient-safety competency and a legal responsibility (Joint Commission; ANA).

Purpose

Build the therapeutic nurse-patient relationship — trust is the foundation of all care.
Gather accurate data — subjective information depends entirely on how the nurse asks and listens.
Provide support and reduce anxiety — helping patients express and process feelings.
Educate patients — teaching only works if the message is received and understood.
Coordinate safe care among the team — structured, accurate handoffs prevent errors.
Advocate for the patient — communicating patient needs and concerns to the team.

The Components of Communication

Every exchange has five parts: a sender, a message, a channel (verbal, written, nonverbal), a receiver, and feedback (confirmation the message was understood). Breakdown at any point distorts the message.

Verbal vs. nonverbal:

Verbal: the words themselves — vocabulary, clarity, pacing, tone.
Nonverbal: body language, facial expression, eye contact, posture, gestures, touch, and personal space. Research consistently shows the majority of communication is nonverbal, and when words and body language conflict, people believe the nonverbal message.

Key point: When a patient says "I'm fine" while crying and avoiding eye contact, the nonverbal message is the true one. The therapeutic response addresses the observed feeling, not the words: "You say you're fine, but you seem tearful. Tell me what's on your mind."

Phases of the Therapeutic Relationship (Peplau)

Hildegard Peplau's model describes four phases (Peplau; Potter & Perry):

Pre-interaction: Before meeting the patient — the nurse reviews the chart and prepares.
Orientation (introductory): First meeting — establish rapport and trust, set the tone, and clarify roles and boundaries. Confidentiality is established here.
Working: The bulk of the relationship — the patient explores problems and works toward goals; the nurse uses therapeutic techniques and provides care.
Termination: Ending the relationship — summarize progress and prepare the patient for the conclusion of care. Begins to be planned well before the final interaction.

Therapeutic Communication Techniques

These techniques encourage the patient to express themselves and keep the focus on them:

Active listening: Full attention — the foundation of all techniques. Often remembered by SOLER: Sit facing the patient, Open posture, Lean in, Eye contact, Relaxed.
Open-ended questions: Invite elaboration ("How have you been feeling since the surgery?") rather than yes/no.
Silence: Gives the patient time to think and signals the nurse is willing to wait — a powerful, underused technique.
Restating/paraphrasing: Repeating the patient's message in your words to confirm understanding.
Reflecting: Directing feelings/questions back ("You're worried about how your family will cope?").
Clarifying: "I'm not sure I understand — can you explain what you mean by 'bad spells'?"
Focusing: Guiding a rambling conversation to the key issue.
Offering self: "I'll sit with you for a while." Presence without an agenda.
Broad opening leads: "What would you like to talk about?"
Summarizing: Reviewing the main points at the end.
Acknowledging feelings / empathy: "This must be very frightening for you."
Providing general information and offering hope realistically (without false reassurance).

Analogy: Silence in therapeutic communication is like the pause in music — the space between notes is what gives the melody meaning. Nurses new to the skill rush to fill silence; experienced nurses let it breathe, and patients fill it with what matters most.

Nontherapeutic Communication (Blocks to Avoid)

These shut down communication and shift focus away from the patient — classic NCLEX wrong answers:

False reassurance: "Everything will be fine." Dismisses real fear and is often untrue.
Giving advice: "If I were you, I'd..." Removes the patient's autonomy.
Asking "why" questions: "Why didn't you take your medication?" Feels accusatory and demands justification.
Closed-ended questions (when exploring feelings): limits the patient to yes/no.
Changing the subject: Signals discomfort and abandons the patient's concern.
Offering approval/disapproval: "That's good" / "You shouldn't feel that way." Judges the patient.
Minimizing/belittling feelings: "Everyone feels that way." Dismisses their experience.
Defensiveness: Defending the staff/facility instead of hearing the concern.
Stereotyping and using clichés: generic, impersonal responses.
Being judgmental or demanding explanations.

Memory aid: If a response starts with "Why...," gives advice, reassures falsely, or makes it about the nurse's opinion, it is almost certainly nontherapeutic — eliminate it on the NCLEX.

SBAR — Professional Team Communication

SBAR standardizes handoffs and provider notifications, reducing the communication errors behind most sentinel events (Joint Commission):

S — Situation: Who you are, the patient, and the immediate problem. "This is Maria, RN on 4 West. Mr. Lee in 412 has a new oxygen saturation of 86%."
B — Background: Relevant history. "He's post-op day 1 from a hip replacement, no prior respiratory disease."
A — Assessment: Your findings and interpretation. "RR 28, crackles at the bases, using accessory muscles; I think he may be developing fluid overload or a PE."
R — Recommendation: What you need. "I'd like you to come evaluate him now, and may I get an order for a stat chest X-ray and ABG?"

Special Communication Situations

Patients with hearing impairment: Face the patient, ensure good lighting, speak clearly at a normal-to-slightly-slower pace (don't shout — it distorts lip reading), reduce background noise, use written materials or a sign-language interpreter as needed.
Patients with visual impairment: Announce your presence and identify yourself, explain before touching, describe the environment.
Aphasia (expressive vs. receptive): For expressive aphasia, give time, use yes/no questions, picture boards; for receptive aphasia, use simple words, gestures, and demonstration.
Non-English-speaking patients: Use a trained medical interpreter — never a family member (especially not a child), for accuracy and confidentiality. Speak to and look at the patient, not the interpreter.
Intubated/nonverbal patients: Use writing tools, communication boards, yes/no signals; anticipate needs.
Cognitively impaired patients (dementia/delirium): Simple, short sentences, one instruction at a time, calm tone, reorient gently, use familiar objects.
Angry patients: Stay calm, maintain a safe distance, listen without defensiveness, acknowledge the feeling, set limits on unacceptable behavior while validating the emotion.
Culturally diverse patients: Be aware that eye contact, personal space, touch, and family involvement norms vary; assess preferences rather than assume.

Normal Findings (Effective Communication)

The patient talks more than the nurse; open-ended questions and silence draw out concerns; the nurse's verbal and nonverbal messages match; feelings are acknowledged rather than dismissed; handoffs are structured (SBAR) and complete; verbal orders and critical values are read back; and interpreters are used appropriately. The patient shows trust — sharing concerns, asking questions, and following through on the plan.

Abnormal Findings (Communication Breakdown)

The nurse dominates the conversation; false reassurance, advice-giving, and "why" questions shut the patient down; the patient becomes withdrawn or noncompliant; nonverbal cues are ignored; handoffs are rushed or incomplete; family members are used as interpreters; and mismatches between words and body language go unaddressed. These are cues of a failing therapeutic relationship and a safety risk.

Causes / Barriers to Communication

Physical barriers (noise, pain, hearing/vision/speech deficits, medications causing sedation), psychological barriers (anxiety, fear, anger, depression, cognitive impairment), cultural/language barriers, environmental barriers (lack of privacy, interruptions), and nurse-related barriers (bias, being rushed, discomfort with emotion, nontherapeutic habits). Recognizing the barrier is the first step to overcoming it.

Signs and Symptoms of Impaired Communication

Withdrawal, silence that signals distress rather than reflection, agitation, frustration, refusal to participate in care, misunderstanding instructions, and nonverbal signs of anxiety or pain (guarding, restlessness, facial tension).

Nursing Assessment

Assess the patient's preferred language and need for an interpreter; hearing, vision, and speech ability; cognitive and emotional state; cultural preferences; health literacy; and readiness to communicate. Observe nonverbal cues and watch for mismatches between words and body language. Assess your own communication habits and biases.

Laboratory / Diagnostic Considerations

Not lab-driven, but physiologic states detected in labs and vitals affect communication: hypoxia, hypo-/hyperglycemia, electrolyte imbalances, and elevated ammonia (hepatic encephalopathy) cause confusion and impaired communication. Correcting the underlying physiologic cause often restores the patient's ability to communicate — always rule out a physiologic reason for a sudden communication change.

Nursing Interventions

Establish trust in the orientation phase; use therapeutic techniques (active listening, open-ended questions, silence, reflection, empathy); avoid nontherapeutic blocks; address nonverbal cues directly; adapt to special communication needs (interpreters, communication boards, sensory aids); use SBAR for all team communication and read-back for verbal orders and critical values; and confirm patient understanding with teach-back. Document communications accurately, especially provider notifications.

Medications affecting communication: Sedatives, opioids, and benzodiazepines slow cognition and speech; anticholinergics and some psychiatric medications cause confusion or dry mouth affecting speech; correcting oversedation may be necessary before meaningful communication or teaching.

Patient Education

Communication is the vehicle of education: assess readiness and learning style first, use plain language (avoid jargon), teach one concept at a time, use visual aids, and confirm with teach-back — the only reliable proof the message was received ("Show me how you'll draw up your insulin"), not "Do you understand?" which patients often answer "yes" out of politeness. Provide written materials at an appropriate literacy level and in the patient's language (AHRQ health literacy guidance).

Safety Considerations

Handoffs and provider notification are the highest-risk moments — use SBAR, minimize interruptions, and confirm receipt of critical information (Joint Commission).
Read-back is mandatory for verbal/telephone orders and critical lab values.
Use trained interpreters, never family/children, to protect accuracy and confidentiality (and to meet legal requirements).
Confidentiality: Communicate about patients only with the care team and only where privacy is maintained (HIPAA).
Speak up: Any team member should voice a safety concern (two-challenge rule); psychological safety saves lives.

NCLEX Clinical Judgment Points (CJMM Mapping)

Recognize cues: Notice nonverbal distress, mismatches between words and body language, and barriers to communication.
Analyze cues: Interpret what the patient is really expressing beneath the words.
Prioritize hypotheses: Determine the patient's primary concern and whether a physiologic cause underlies impaired communication.
Generate solutions: Select the therapeutic technique or communication aid that fits the situation.
Take actions: Respond therapeutically; use SBAR and read-back for the team.
Evaluate outcomes: The patient expresses feelings, shows understanding (teach-back), and trust is maintained.

Common NCLEX Traps

Choosing false reassurance ("Don't worry, everything will be fine") — always nontherapeutic.
Selecting advice-giving ("What I would do is...") — removes patient autonomy.
Picking a "why" question — sounds accusatory and demands justification.
Closing down feelings with a yes/no question when the stem calls for exploring emotion.
Responding to the words instead of the observed feeling — the therapeutic answer addresses the nonverbal cue.
Using a family member as interpreter — always choose the trained medical interpreter.
Shouting at a hearing-impaired patient — face them, speak clearly at a normal pace instead.

Offering false reassurance to ease their own discomfort with the patient's distress.
Giving advice ("If I were you...") instead of supporting the patient's own decision-making.

Filling silence too quickly instead of letting the patient think and respond.

Scenario 1 (Therapeutic response): A newly diagnosed cancer patient says, "I don't know how I'm going to get through this." Which response is most therapeutic?
