Why Assessment Matters

Patient safety: the postpartum period is a time of rapid change and real risk. The leading danger is postpartum hemorrhage (usually from a uterus that won't contract), followed by infection, VTE (clots), and mental-health emergencies. Frequent, systematic assessment catches these early.

Early recognition: the earliest sign of hemorrhage is often a boggy (soft) fundus and increasing lochia—before vital signs change (young mothers compensate well, then crash). The nurse who checks the fundus catches it first.

Prevention of complications: recognizing a boggy uterus, a full bladder displacing the fundus, saturating lochia, calf pain/swelling (VTE), or signs of infection/depression and acting quickly prevents shock, clots, and crises.

Clinical judgment: the nurse integrates the fundus, lochia, vitals, and emotional status to decide what's normal recovery, what's abnormal, and what needs immediate action.

Connecting to A&P Recall that after birth the uterus must contract to clamp the vessels where the placenta detached, and that oxytocin drives that contraction and milk let-down. Assessment checks whether the uterus is contracting (firm fundus) and involuting normally, and whether recovery is on track across all systems (BUBBLE-HE).

NCLEX pearl — know who is at high risk for hemorrhage Postpartum hemorrhage risk rises with an overdistended uterus (large baby, twins, polyhydramnios), prolonged or precipitous labor, grand multiparity, retained placenta, and a full bladder. Knowing the risk factors tells you which mothers to watch most closely for a boggy fundus.

Inspection

What the nurse observes: lochia (amount/color on the pad and under the buttocks), the perineum/incision (REEDA), breasts/nipples, legs (edema/redness), and the mother's affect/interaction with the newborn.

Why "check under the buttocks" matters for lochia Lochia can pool under the buttocks and lower back while the pad looks only lightly soiled—so blood loss is underestimated. Always turn the client and check underneath when assessing lochia. Saturating a pad in an hour or less is a sign of hemorrhage.

Palpation

Technique: palpate the fundus—with the client supine and knees slightly flexed, support the lower uterus with one hand just above the symphysis while palpating the fundus with the other. Assess tone (firm vs. boggy), height (in relation to the umbilicus), and position (midline vs. displaced). Also palpate the bladder, breasts, and calves.

Safety — a boggy fundus is the first sign of hemorrhage A boggy (soft) uterus means it isn't clamping the placental vessels—the most common cause of postpartum hemorrhage (uterine atony). The first nursing action is to firmly massage the fundus until it firms up; also empty the bladder (a full bladder pushes the uterus up and to the side and prevents contraction). If bogginess/bleeding continues, give oxytocin/uterotonics and notify the provider.

Percussion

Purpose: percussion is limited postpartum. It can help assess a distended bladder (dullness suprapubically) if urinary retention is suspected—otherwise the assessment relies on inspection and palpation.

Auscultation

Technique: auscultate bowel sounds (especially after cesarean—return of bowel function) and the heart/lungs as part of routine care; monitor vital signs for bleeding/infection.

Why tachycardia is an ominous late sign of hemorrhage A young, healthy postpartum mother compensates for blood loss by increasing her heart rate before her blood pressure drops—so rising tachycardia (and later hypotension) can be a late sign that she's already lost a lot of blood. That's why the nurse relies on the fundus and lochia (early signs) rather than waiting for vital-sign changes.

BUBBLE-HE is the head-to-toe postpartum check. The fundus should be firm, midline, and at/near the umbilicus, descending ~1 fingerbreadth per day (involution). Lochia progresses rubra → serosa → alba; a return to bright red or saturating flow is abnormal.

Uterus (fundus): tone (firm vs. boggy), height (umbilicus/descent), and position (midline)—support the lower uterus while palpating. ↓ 4. Documentation tip Record the fundus (tone/height/position), lochia (type/amount/clots), perineum (REEDA), voiding, VS, emotional status, and any interventions with times and provider notification. Describe fundal height in fingerbreadths above/below the umbilicus (e.g., "U−1") and lochia amount objectively (scant/light/moderate/heavy or grams if weighed).

Clinical Judgment

Recognize cues: two hours after a vaginal birth of a large baby, the nurse finds the fundus boggy and displaced above and to the right of the umbilicus, with heavy lochia rubra (a saturated pad). The client says she "hasn't been able to pee."
Analyze cues: a boggy, displaced fundus + heavy lochia + a full bladder = uterine atony (postpartum hemorrhage risk), worsened by the full bladder preventing uterine contraction and by the overdistended uterus (large baby).
Prioritize problems: make the uterus contract and control bleeding—this is the priority to prevent hemorrhage/shock.

Recognize & analyze cues

Cues: boggy, displaced (right/high) fundus + heavy lochia + no voiding + mild tachycardia after a large baby and long labor = uterine atony with early postpartum hemorrhage, worsened by a full bladder. Risk: hypovolemic shock.

Prioritize problems

Priority: make the uterus contract and stop the bleeding. This is urgent—the tachycardia suggests she's already compensating for blood loss.

Determine next steps

Massage the fundus first (it should firm up), help her empty the bladder (or catheterize), give oxytocin/uterotonics as ordered, monitor VS and lochia, ensure IV access, and notify the provider. Reassess the fundus and bleeding frequently.

Evaluate
