Definition

The ABCs are a rapid prioritization framework used to identify and sequence care for life-threatening physiologic problems: Airway patency first, effectiveness of Breathing second, adequacy of Circulation third. It is a decision-making hierarchy, not an assessment checklist — its purpose is to answer "what will kill this patient first?" (Ignatavicius et al., Elsevier; Silvestri, Elsevier).

Purpose

Why airway before breathing before circulation? It follows the physiology of oxygen delivery: oxygen must (1) enter through an open airway, (2) be moved into the alveoli and blood by ventilation, and (3) be delivered to tissues by circulation. A failure upstream makes everything downstream irrelevant — perfect lungs and a strong heart cannot compensate for a blocked trachea. Brain cells begin dying after roughly 4–6 minutes without oxygen, which is why airway compromise is the most time-critical emergency (AHA; Ignatavicius, Elsevier).

House analogy: a broken pipe (circulation) floods a room over hours; a blocked front door during a fire (airway) kills everyone in minutes.

On the NCLEX, the ABCs are one of three core prioritization tools, alongside Maslow's hierarchy and the nursing process, and they map directly onto the CJMM steps of prioritize hypotheses and take action (NCSBN 2026 NCLEX-RN Test Plan).

Pathophysiology of Each Letter

A — Airway (the front door). The passage from nose/mouth to trachea. Obstruction can be mechanical (tongue — the most common cause in an unconscious patient — food, blood, vomitus, tumor) or from tissue swelling (anaphylaxis, burns/inhalation injury, angioedema, post-extubation laryngeal edema). Obstruction stops oxygen at the door.

B — Breathing (air moving through the house). Ventilation and gas exchange. The airway can be wide open while breathing still fails: opioid overdose suppresses the respiratory drive, pneumothorax collapses a lung, severe asthma traps air, Guillain-Barré or high spinal injury paralyzes respiratory muscles.

C — Circulation (the plumbing). Delivery of oxygenated blood to tissue. Fails from pump problems (MI, heart failure, lethal dysrhythmias), volume problems (hemorrhage, dehydration), or distribution problems (sepsis, anaphylactic and neurogenic shock).

Some sources add D (Disability — neuro status/glucose) and E (Exposure); NCLEX questions center on A-B-C.

Two testable patterns here. First, restlessness, agitation, and anxiety are often the earliest signs of hypoxia — before SpO2 drops. Students expect cyanosis; cyanosis is late. Second, hypotension is a late sign of shock — the body compensates with tachycardia and vasoconstriction first, especially in young patients (Ignatavicius, Elsevier; ATI).

Causes and Signs & Symptoms — High-Yield Emergencies

Airway threats: anaphylaxis (stridor + hives + hypotension after new drug/food/bee sting), inhalation burns (facial burns, singed nasal hair, hoarse voice — intubate early because swelling worsens over hours), epiglottitis (tripod position, drooling — do NOT inspect the throat), post-thyroidectomy hematoma (keep trach set at bedside).

Breathing threats: tension pneumothorax (tracheal deviation, absent breath sounds one side), opioid-induced respiratory depression (RR <8, pinpoint pupils → naloxone), pulmonary embolism (sudden dyspnea + chest pain + tachycardia, classically post-op or with DVT), flail chest, severe asthma progressing to a silent chest (no wheeze because no air is moving — deterioration, not improvement).

Circulation threats: hemorrhage (check under the patient — blood pools), MI, lethal rhythms (VF, pulseless VT), sepsis, dehydration in the very young and very old.

Nursing Assessment

Assess in ABC order, fastest check first. Ask the patient a question: a patient who answers you in a full, clear sentence has demonstrated a patent airway, adequate breathing, and enough perfusion to stay conscious — all in five seconds. Then: look/listen for obstruction sounds (stridor, snoring, gurgling); count respiratory rate for a full minute if irregular, watch work of breathing, auscultate, check SpO2; palpate pulses, check skin color/temperature/cap refill, BP, LOC, urine output (the kidneys are the "canary" of perfusion — output <0.5 mL/kg/hr signals trouble).

Clinical reasoning link (CJMM): the assessment findings are your cues; the ABC framework is how you prioritize hypotheses among them.

Laboratory Findings & Diagnostic Tests

Labs that map to each letter: ABGs (falling PaO2, rising PaCO2 = ventilatory failure; a "normalizing" CO2 in an exhausted asthmatic is deterioration, not recovery), lactate >2–4 mmol/L (tissue hypoperfusion — key sepsis marker, per Surviving Sepsis/AHRQ-endorsed bundles), hemoglobin/hematocrit (hemorrhage — but H&H may be falsely normal immediately after acute bleeding before hemodilution), troponin (MI), and capnography (gold standard for confirming ventilation). Diagnostics: chest X-ray, ECG, continuous pulse oximetry. Remember: treat the patient, not the monitor — never delay intervention for a deteriorating patient to wait for lab confirmation.

Nursing Interventions (in priority order)

Airway: position first — head-tilt/chin-lift, or jaw thrust if cervical spine injury is suspected; suction visible secretions; insert oral airway only if unconscious (gag reflex); anticipate intubation for progressive swelling. Positioning is the highest-yield, zero-equipment intervention — on the NCLEX, "reposition the patient" frequently beats "call the provider."

Breathing: raise the head of bed (high-Fowler's for dyspnea), apply oxygen (in an emergency you do not need an order to apply oxygen to a hypoxic patient — safety first, per standard of care), give reversal agents (naloxone) or rescue meds (bronchodilators, epinephrine for anaphylaxis) per protocol, prepare for ventilatory support.

Circulation: control external bleeding with direct pressure, establish/verify IV access, give fluids per protocol, place patient supine with legs elevated for hypovolemia, initiate CPR and defibrillation for pulseless rhythms (AHA).

Always pair with: rapid response activation for deterioration — Joint Commission patient safety goals support early escalation rather than waiting.

Patient Education

Teach families of at-risk patients: signs of anaphylaxis and EpiPen use, aspiration precautions for dysphagia (upright for meals, 30–60 min after), opioid safety at home (naloxone availability, never mix with alcohol/benzodiazepines — CDC opioid guidance), when to call 911 (any stridor, blue lips, severe dyspnea, chest pain).

Safety Considerations

The ABCs exist because of patient safety: failure to rescue — not recognizing deterioration in time — is a leading cause of preventable inpatient death (AHRQ). The nurse's job is recognizing the early, subtle cues (restlessness, rising RR, falling urine output) before the late, obvious ones. Respiratory rate is the most frequently omitted vital sign and often the first to change.

NCLEX Clinical Judgment Points

How to actually use ABCs on a question: (1) ABCs apply when comparing physiologic threats — "which patient do you see first?" (2) An actual problem usually outranks a potential one, but an unstable "at risk" airway (e.g., new stridor) beats a stable actual problem. (3) Unexpected findings outrank expected ones: a COPD patient with SpO2 90% is expected; a post-op patient newly at 90% is not — see the post-op patient. (4) A compromised airway beats everything, including pain — pain is distressing but not lethal. (5) When two patients both have "C" problems, pick the more unstable/less compensated one.

Common NCLEX Traps

The biggest trap: CPR uses C-A-B, not A-B-C. Since 2010, AHA resuscitation guidelines sequence compressions first in cardiac arrest, because circulating existing oxygen matters most once the heart stops. ABC is for the living, deteriorating patient; C-A-B is for the pulseless one (AHA).

Other traps: choosing the loud patient over the quiet one (the screaming patient has an airway; the silent one may not); picking cyanosis as an "early" sign; assuming airway is fine because O2 is flowing (a nasal cannula doesn't open a blocked airway); choosing "notify the provider" when a nursing action (position, suction, oxygen) can fix the problem now; and forgetting that a patient who "suddenly became calm/sleepy" after respiratory distress is likely tiring out and about to arrest.

Five NCLEX Clinical Pearls

A patient talking in full sentences has just proven A, B, and C are (currently) intact.
Restlessness = hypoxia until proven otherwise; check oxygenation before assuming pain or anxiety.
Stridor and drooling = impending airway obstruction = see this patient first, almost always.
A "normal" CO2 in a struggling asthmatic and a "suddenly quiet" distressed patient both mean deterioration.
Cardiac arrest = C-A-B (compressions first); everything else = A-B-C.

Five Common Student Mistakes

Applying ABC mechanically without asking "is this finding expected for this patient?"
Treating a potential airway problem (new stridor) as lower priority than an actual stable problem.
Selecting "call the provider/rapid response" when an immediate nursing intervention is an option.
Equating oxygen saturation with airway patency.
Ranking severe pain above a physiologic threat because it's "a 10/10."

Five Application Scenarios

Post-thyroidectomy, the patient reports neck tightness; the dressing is intact but her voice is hoarse and she's swallowing constantly. Priority? → Airway — hematoma compressing the trachea. Assess, call for help, keep trach/suture removal set at bedside.
Four patients: (a) chest pain 6/10 with history of GERD, (b) asthmatic whose wheezing just stopped and is drowsy, (c) post-op with 50 mL bright red drainage, (d) diabetic with glucose 250. Who first? → (b): silent chest + drowsiness = impending respiratory arrest.
Patient on PCA morphine: RR 6, sedated, pinpoint pupils. First action? → Stop the PCA, stimulate, support breathing, give naloxone per protocol (Breathing).
Trauma patient, suspected C-spine injury, snoring respirations. Airway maneuver? → Jaw thrust, not head-tilt/chin-lift.
Patient found pulseless and apneic. Sequence? → C-A-B: start compressions, then airway, then breaths; defibrillate as soon as available.
