Purpose (Why This Matters for the Nurse)

The nurse is the person who monitors the patient over time and is most likely to catch an adverse effect first. Recognizing early warning signs lets the nurse intervene before harm becomes irreversible — holding the drug, notifying the provider, giving an antidote, and preventing the reaction from progressing. Understanding adverse effects also drives accurate patient teaching so people know which symptoms to report.

Allergic Reactions and Anaphylaxis

An allergic reaction requires prior exposure that sensitized the immune system; the reaction occurs on a later exposure. Severity ranges widely:

Mild: localized rash, itching (pruritus), hives (urticaria), watery eyes.
Moderate: widespread hives, facial swelling, wheezing.
Severe — anaphylaxis (EMERGENCY): rapid onset of airway swelling, bronchospasm, stridor, hypotension, tachycardia, and shock. This is life-threatening.

⚠ Anaphylaxis priority (ABCs): stop the drug immediately, call for help/Rapid Response, maintain the airway and give oxygen, and anticipate epinephrine (first-line), followed by antihistamines, corticosteroids, and IV fluids. Airway and breathing come first.

Severe skin reactions such as Stevens-Johnson syndrome (blistering, mucosal involvement, skin sloughing) are dermatologic emergencies — stop the drug and notify the provider at once.

A black box (boxed) warning is the FDA’s strongest alert about a serious or life-threatening risk — for example, increased suicidality in young patients starting antidepressants, or serious bleeding with anticoagulants. The nurse should recognize these and teach accordingly.

Teratogenic drugs harm a developing fetus and must be avoided in pregnancy — examples include warfarin, ACE inhibitors/ARBs, isotretinoin, and certain anticonvulsants. Always confirm pregnancy status before administering high-risk drugs.
