Why This Body System Matters

Primary functions: pregnancy is a team effort between the mother and the placenta to grow a baby. The placenta exchanges oxygen, nutrients, and wastes and makes the hormones that sustain the pregnancy; the uterus houses and protects the fetus and then contracts to deliver it; and the mother's body undergoes dramatic adaptations to support two lives.

Homeostasis: the maternal body increases blood volume, cardiac output, and oxygen delivery to supply the fetus, while the placenta keeps the fetus fed and oxygenated. When any part fails (placental problems, hypertension, bleeding), both patients are threatened.

Why nurses assess it frequently: nurses monitor fundal height, fetal heart rate, blood pressure, fluid status, and labor progress—all of which depend on this physiology. Understanding the normal changes lets you spot the abnormal.

Why it's heavily tested on the NCLEX: maternity content ties together the placenta/hormones, fetal circulation, maternal adaptations, labor, and complications. If you understand the physiology, OB assessments, medications, and emergencies become logical.

Big picture Think of pregnancy as building a life-support system inside the uterus. The placenta is the "life-support machine"—it breathes, feeds, and cleans the blood for the fetus and runs the hormone "control panel." The umbilical cord is the cable, the amniotic fluid is the protective cushion, and the mother's body is the power plant that ramps up to supply it all.

The structures of pregnancy

Fetal circulation (why it's different)

Ductus venosus	Shunts blood past the fetal liver (the placenta already processes it).
Foramen ovale	An opening that lets blood cross from the right atrium to the left atrium—bypassing the non-working lungs.
Ductus arteriosus	Connects the pulmonary artery to the aorta, shunting blood away from the lungs.

NCLEX pearl — the cord and the shunts The umbilical cord has 2 arteries and 1 vein; the vein carries oxygenated blood to the fetus. The fetus doesn't use its lungs, so three shunts (ductus venosus, foramen ovale, ductus arteriosus) bypass them—they close after birth when the newborn breathes (a shunt that stays open = a congenital heart problem, e.g., patent ductus arteriosus).

Physiology

From conception to a working placenta

1. Fertilization: sperm meets egg in the fallopian tube; the fertilized egg (zygote) begins dividing as it travels toward the uterus. ↓ 2. Implantation: the developing embryo implants in the uterine lining (endometrium) ~6–10 days after fertilization; the cells that will become the placenta start making hCG (which keeps the pregnancy going and shows up on pregnancy tests). ↓ 3. Placenta forms: the placenta develops and takes over as the fetal lungs, gut, kidneys, and hormone factory. Assessment link: a rising hCG confirms early pregnancy. ↓ 4. Fetal growth: the placenta delivers oxygen and nutrients and removes wastes across the umbilical cord while the fetus develops in the cushioning amniotic fluid.

The hormones of pregnancy

Estrogen	Promotes uterine/breast growth and increases blood flow; contributes to fluid retention.
Human placental lactogen (hPL)	Alters maternal metabolism to supply glucose to the fetus—raises maternal insulin resistance (contributes to gestational diabetes).
Oxytocin	Stimulates uterine contractions (labor) and the milk let-down reflex.
Prolactin	Stimulates milk production for breastfeeding.

Maternal adaptations (why the "normal" changes)

Cardiovascular	Blood volume ↑ ~40–50%, cardiac output ↑, HR ↑ to supply the fetus; a slight physiologic anemia occurs (plasma rises more than red cells). BP dips mid-pregnancy. Lying flat can compress the vena cava → supine hypotension (turn to the left side).
Respiratory	Oxygen demand and tidal volume ↑; the growing uterus pushes up the diaphragm → dyspnea is common.
Renal	GFR and renal blood flow ↑; more urinary frequency (and UTI risk); mild glucose/protein spillage can occur.
GI	Progesterone slows motility → nausea, constipation, and reflux/heartburn.
Musculoskeletal/other	Ligaments loosen (relaxin) → waddling gait/back pain; increased clotting factors → higher clot (VTE) risk.

Why turning to the LEFT side matters When a pregnant woman lies flat on her back, the heavy uterus can compress the inferior vena cava, reducing blood return to the heart → supine hypotensive syndrome (dizziness, low BP, and reduced blood flow to the placenta). Turning her to the left lateral position takes the weight off the vena cava and improves circulation to both mother and fetus. This is a frequently tested, simple, life-improving intervention.

Homeostasis

Balance	How the maternal-fetal unit maintains it
Oxygen/nutrient delivery	The mother's increased blood volume and cardiac output plus the placenta keep the fetus supplied.
Fluid balance	Increased blood volume and renal changes support the pregnancy; too much/too little amniotic fluid signals problems.
Glucose supply	hPL shifts maternal metabolism to prioritize fetal glucose (at the cost of maternal insulin resistance).
Pregnancy maintenance	hCG then progesterone keep the uterus quiet and the lining intact until term.
Labor & postpartum	Oxytocin drives contractions and, after birth, clamps uterine vessels to prevent hemorrhage.

Feedback mechanisms

Labor is a classic positive-feedback loop (rare in the body). As the fetus presses on the cervix, the body releases oxytocin → stronger contractions → more cervical pressure → more oxytocin—the cycle builds until birth, then stops. Breastfeeding is another positive-feedback loop: suckling → prolactin (milk production) and oxytocin (let-down) → more milk → more suckling.

Feedback in plain language Most body loops shut off the trigger (negative feedback). Labor is the opposite: each contraction makes the next one stronger (positive feedback) until the baby is born. That's why oxytocin (Pitocin) is used to start/strengthen labor—and why it's watched closely.

Interactions With Other Body Systems

Cardiovascular	Massive increases in blood volume and cardiac output supply the fetus; pregnancy stresses the heart and raises clot risk. Placental blood flow depends on maternal BP.
Respiratory	Oxygen demand rises; the mother breathes for two—and the placenta does the fetal "breathing."
Endocrine	Pregnancy is largely hormone-driven (hCG, estrogen, progesterone, hPL, oxytocin, prolactin); hPL causes insulin resistance (gestational diabetes).

Immune	The maternal immune system tolerates the fetus (which is genetically half "foreign") while still fighting infection; some immunity passes to the baby.

Clinical Connections

You'll study these later—here's simply how disrupted physiology leads to disease (don't memorize the diseases yet, just the logic):

Ectopic pregnancy: implantation outside the uterus (usually the tube)—can rupture and hemorrhage.
Gestational hypertension/preeclampsia: abnormal placental development → vasospasm, high BP, and organ/placental hypoperfusion.
Gestational diabetes: hPL-driven insulin resistance outpaces the mother's insulin.
Placenta previa/abruptio: a misplaced or prematurely detaching placenta → bleeding.
Preterm labor / PROM: the uterus contracts or membranes rupture too early.
Postpartum hemorrhage: the uterus fails to contract (atony) after birth.
Hyperemesis gravidarum: severe hCG-related nausea/vomiting → dehydration.
