Why Nurses Review Laboratory Tests

Labs give objective data about what's happening inside the body that a physical exam can't show. Providers order them to screen, diagnose, monitor disease progression, evaluate treatment response, and check for medication toxicity (e.g., digoxin levels, vancomycin troughs). Why Values Are Never Interpreted in Isolation

A lab value is one puzzle piece, not the whole picture. Analogy: a lab result is like a fuel gauge reading — "half tank" means something different if you're pulling into your driveway versus starting a cross-country trip. Always pair the number with the patient's assessment, history, medications, and other labs.

Example: K+ of 3.4 in a healthy adult may just need oral replacement. The same K+ of 3.4 in a patient on digoxin is dangerous — hypokalemia potentiates digoxin toxicity.

NCLEX rule: treat the patient, not the number.

Trends vs. Single Results

One result is a snapshot; a trend is the movie. A hemoglobin of 9.0 that's been stable for months (chronic kidney disease) is expected. A hemoglobin that dropped from 12 to 9 overnight suggests active bleeding — an emergency. Always compare to the patient's baseline and previous results before acting. Normal vs. Abnormal vs. Critical Values

Normal: within reference range for that lab/population. Document and continue monitoring.
Abnormal: outside range but not immediately life-threatening. Assess the patient, look at trends, notify provider per protocol.
Critical (panic) value: a result so far outside range it poses immediate risk of death without intervention (e.g., K+ 6.8, glucose 28, platelets 8,000). The Joint Commission (NPSG 02.03.01) requires timely reporting of critical results to the responsible provider — assess the patient first, then notify immediately, and document both.

When Abnormal Values Require Immediate Intervention

Intervene immediately when the abnormality threatens airway, breathing, circulation, or neuro status:

K+ >6.0 or <2.5 (lethal dysrhythmias)
Glucose <50 or >400–500 (neuro compromise, DKA/HHS)
Na+ <120 or >160 (seizures, cerebral edema)
Hgb <7 with symptoms (tissue hypoxia)
Platelets <20,000 (spontaneous bleeding)
ABG pH <7.25 or >7.55
Elevated troponin with chest pain (MI)
Ca2+ <7 (tetany, laryngospasm)

When to Notify the Provider Immediately

Notify immediately for: any critical value, a new abnormal value with symptoms (e.g., low K+ plus irregular pulse), a rapidly worsening trend, a value affecting a scheduled medication (hold digoxin if K+ low; hold heparin if aPTT critically high), or lab results conflicting with a current order. Use SBAR: have the value, the trend, vital signs, and your assessment ready.

Urgent Action vs. Continued Monitoring

Urgent action: critical values, symptomatic abnormalities, unstable vitals, values affecting high-alert drugs (insulin, anticoagulants, digoxin, potassium).
Continued monitoring: mildly abnormal values consistent with the patient's known chronic condition and stable baseline (e.g., creatinine 2.0 in known CKD; BUN 24 in mild dehydration already being treated).

The dividing line is clinical judgment: Is the patient symptomatic? Is it new? Is it trending worse? Yes to any = act.

Correlating Labs with Physical Assessment

Labs should confirm what your hands, eyes, and stethoscope find — or prompt you to look again:

Low Hgb → pallor, tachycardia, fatigue, dyspnea
High K+ → muscle weakness, peaked T waves, bradycardia
Low Na+ → confusion, headache, seizures
High WBC → fever, warmth, purulent drainage
Elevated BUN/creatinine → decreased urine output, edema
Low albumin → edema (fluid shifts out of vessels)

If the lab and the patient don't match (K+ 7.0 in an asymptomatic patient with a difficult, hemolyzed draw), suspect specimen error and request a redraw — but never assume; assess first.

How Medications Influence Lab Values

Diuretics (loops/thiazides): ↓K+, ↓Na+; spironolactone ↑K+
ACE inhibitors/ARBs: ↑K+, ↑creatinine
Corticosteroids: ↑glucose, ↑WBC, ↓K+
Warfarin: ↑PT/INR; Heparin: ↑aPTT
Statins, acetaminophen, isoniazid: ↑AST/ALT
Aminoglycosides, vancomycin, NSAIDs, contrast dye: ↑BUN/creatinine (nephrotoxic)

Recognize cues: Is the value abnormal? Critical?
Analyze cues: Compare to baseline/trend; correlate with assessment, meds, diagnosis.
Prioritize hypotheses: What's the most likely and most dangerous explanation? (ABCs first)
Generate solutions: Redraw? Notify? Hold a med? Implement protocol?
Take action: Assess patient → intervene → notify provider → document.
Evaluate outcomes: Recheck labs and reassess.

Priority Nursing Interventions by Finding

Critical K+ (high): cardiac monitor, hold K+ sources, anticipate calcium gluconate/insulin+dextrose/kayexalate
Critical glucose (low): 15g fast carbs if alert; IV D50 or glucagon if not
Low platelets/high INR: bleeding precautions — no IM injections, soft toothbrush, fall precautions, hold anticoagulants
Low WBC (neutropenia): protective precautions, monitor temp — fever of 100.4°F is an emergency
Low Hgb: O2, monitor vitals, anticipate transfusion, check for bleeding source
Rising creatinine: strict I&O, review/hold nephrotoxins, monitor K+

Documentation Considerations

Document: the value, date/time you received it, your assessment findings, provider name and exact notification time, orders received, interventions and patient response, and read-back verification for verbal critical value reports. "Dr. Smith notified at 1420 of K+ 6.2; orders received" — not just "MD aware."

Patient Education

Explain the test's purpose in plain language, fasting requirements (e.g., lipids, fasting glucose), medication timing (hold or take before draw — e.g., trough levels drawn before the dose), what to expect, and when/how they'll get results. For ongoing therapy (warfarin/INR, HbA1c), teach why regular monitoring matters and signs to report (bleeding, hypoglycemia).

Common NCLEX Question Patterns

"Which lab value requires immediate notification?" → pick the critical value affecting ABCs
"Which patient should the nurse assess first?" → the one whose lab + symptom combination is most lethal
"Which medication should the nurse hold?" → check the lab that drug affects (digoxin/K+, warfarin/INR, metformin/creatinine)
"Which finding indicates treatment is effective?" → the value trending toward normal
Traps: choosing "notify the provider" before assessing the patient; treating a lone abnormal number in a stable patient; ignoring the medication list in the stem
