What it is

Clinical judgment is the decision or action a nurse takes after weighing all the patient's information to keep them safe. It's the end result of thinking — not just knowing facts, but using them for a specific patient.

Analogy: It's like a GPS re-routing you in real time. Knowing the roads is book knowledge; reading live traffic is clinical reasoning; the actual "turn left now" is the clinical judgment.

This is the whole point of the Next Generation NCLEX (launched April 2023) — it tests whether you can use knowledge, not just recall it.

Three terms students mix up

Critical thinking – your general reasoning skill (the mental toolkit).
Clinical reasoning – applying that thinking to one patient's data in the moment.
Clinical judgment – the actual decision or action you land on.

And the nursing process (ADPIE) — Assessment, Diagnosis, Planning, Implementation, Evaluation — is the step-by-step structure you use to get there.

Analogy: Critical thinking is your driving skill, clinical reasoning is reading the road right now, ADPIE is the traffic laws, and clinical judgment is the decision to brake or turn.

Tanner's 4 phases (the foundation)

Noticing – catching the important cues, even subtle ones.
Interpreting – figuring out what those cues mean.
Responding – taking action.
Reflecting – checking how the patient responded and learning from it.

It's not a straight line — nurses move back and forth as new information comes in.

The NCSBN model: 6 cognitive skills

This is what the NCLEX actually scores. The six steps flow in order:

Recognize Cues – spot the relevant and abnormal findings.
Analyze Cues – decide what the findings mean.
Prioritize Hypotheses – rank what's most likely and most urgent.
Generate Solutions – plan the possible interventions.
Take Actions – carry out the top-priority intervention.
Evaluate Outcomes – check whether it worked and adjust.

Memory hook: Recognize, Analyze, Prioritize, Generate, Take action, Evaluate → "Really Angry Patients Get Totally Evaluated."

How the three frameworks connect

They're the same thinking at different zoom levels:

How to prioritize

ABCs – Airway, Breathing, Circulation come first, always.
Maslow – physical needs before emotional ones.
Acute over chronic, unstable over stable – new or sudden changes usually win.

Analogy: ABCs are the airline rule — oxygen mask on first. Nothing else helps a patient who can't breathe.

Test-taking traps to avoid

Assessing when you should act, or acting when you should assess. Unstable = act. Stable or unsure = assess.
Treating the number instead of the patient. Always read a value against the whole picture.
Ignoring the trend. A potassium climbing 4.0 → 5.6 → 6.4 matters more than one 5.6.
Choosing a "correct" answer that isn't the priority answer.
Picking actions outside the nurse's scope or needing an order.
