Definition

The Five Rights of Delegation are a decision-making framework developed by the NCSBN and endorsed jointly with the ANA that a licensed nurse must apply before, during, and after delegating any nursing task: right task, right circumstance, right person, right directions and communication, right supervision and evaluation (NCSBN, 2019; ANA & NCSBN, 2019). Delegation itself is defined as allowing a delegatee to perform a specific nursing activity that is beyond the delegatee's traditional role but within the delegator's scope — while the delegator retains accountability for the delegation decision.

Analogy: Think of the Five Rights like the "five rights of medication administration," but the "drug" being administered is a task. Just as you never give a med without checking drug–dose–route–time–patient, you never hand off a task without checking task–circumstance–person–communication–follow-up. Skipping one right is how "task errors" happen, exactly like med errors.

Purpose

Delegation is a high-risk clinical judgment, not a workload shortcut. The Five Rights exist to protect patients from receiving care by someone unprepared to give it, and to protect the nurse's license by structuring the decision. NCLEX tests this framework constantly — nearly every "which patient/task do you assign" question is secretly a Five Rights question.

Foundational Framework (in place of Pathophysiology)

Delegation transfers the performance of a task, never the accountability for the decision or the nursing process. Three distinct concepts anchor this lesson:

Delegation: authorizing a delegatee to perform an activity beyond their traditional role but within the delegator's scope (e.g., in states that allow it, RN delegating a specific trained task to a certified medication aide).
Assignment: distributing routine care already within the delegatee's own scope and role (e.g., assigning a stable patient's hygiene to UAP). Most NCLEX items technically involve assignment, but the Five Rights logic applies to both.
Supervision: the active oversight — directing, following up, evaluating — that continues after the handoff.

Accountability is layered, not transferred: the delegating nurse answers for the decision, communication, and follow-up; the delegatee answers for accepting and correctly performing the task; the organization answers for policies, training, and competency validation (NCSBN, 2019).

Normal Findings — What Correct Delegation Looks Like (the Five Rights in detail)

1. Right Task. The task is delegable when it is routine, frequently recurring, performed according to an established procedure, carries predictable results, and does not require nursing judgment, assessment, or evaluation during performance. It must be permitted by the NPA, facility policy, and the delegatee's validated competence. Classic delegable tasks: vital signs on stable patients, hygiene, ambulation, I&O, feeding patients without swallowing risk.

2. Right Circumstance. Even a normally delegable task becomes non-delegable if the patient's condition is wrong for it. The patient must be stable with a predictable outcome; the setting must have adequate resources and supervision available. Vital signs are UAP-appropriate on a stable patient — and RN-only during a blood transfusion's first 15 minutes, immediately post-op, or when the last set was abnormal. The circumstance can revoke the task.

3. Right Person. Right delegator (licensed, authorized by NPA to delegate), right delegatee (scope, training, documented and validated competency — not "she's done it before, probably"), right patient. Competency is individual: two UAPs with identical job titles may not have identical validated skills.

4. Right Directions and Communication. Clear, complete, concise, specific to this patient and this time: what to do, how, when, expected results, and — critically — what to report and by when ("Report a systolic below 100 or above 160 to me immediately"). Two-way: the delegatee must have the chance to ask questions and must not be expected to interpret, decide, or independently modify. "Let me know if anything seems off" fails this right; it delegates judgment.

5. Right Supervision and Evaluation. The delegating nurse monitors performance, remains available, intervenes if needed, obtains and evaluates results, evaluates the patient's outcome, and gives feedback to the delegatee. Delegation is not complete at handoff — it is complete at evaluation. This right is the one most often skipped in practice and most often tested on NCLEX.

Abnormal Findings — What Delegation Should NEVER Include

Never delegable regardless of setting or staffing: initial and ongoing assessment requiring judgment, formulation of nursing diagnoses, care planning, evaluation of outcomes, initial patient teaching, and clinical judgment itself. Also abnormal: delegating to meet staffing convenience rather than patient need, delegating without knowing the delegatee's validated competence, and "delegation" with no plan for follow-up.

Causes (of failed delegation)

Time pressure and understaffing (skipping communication and supervision), assumption of competence from job title, culture of task-dumping, fear of appearing "bossy" leading to vague directions, and unclear facility policies about who may do what.

Signs and Symptoms (of a delegation breakdown)

Abnormal vitals discovered hours later in the chart with no report made; a UAP performing a task she was never trained on; a delegatee saying "no one told me what to watch for"; an RN who cannot describe the outcome of a task she delegated. Each maps to a specific violated Right — that mapping is a favorite NCLEX exercise.

Nursing Assessment — Before Delegating, Assess Three Things

The patient (stable? predictable? recently changed?), the delegatee (scope, validated competence, current workload), and the situation (supervision available? resources adequate?). If any assessment fails, perform the task yourself or reassign.

Regulatory Sources (in place of Laboratory Findings / Diagnostic Tests)

State NPA (some states restrict or define LPN delegation authority; some regulate delegation to medication aides), NCSBN National Guidelines for Nursing Delegation (2019), ANA–NCSBN Joint Statement (2019), ANA Scope and Standards (2021), and facility policy, which may restrict but never expand what the NPA allows.

Nursing Interventions — Applying the Five Rights Step by Step

Work the rights in order, as a checklist: (1) Is this task delegable at all? (2) Is it delegable for this patient right now? (3) Is this specific person authorized and validated to do it? (4) Deliver specific directions with reporting parameters and confirm understanding. (5) Follow up: check performance, obtain results, evaluate the patient, document, give feedback. If the patient's condition changes mid-task, the delegating nurse reassumes care immediately.

Patient Education

Patients may ask "why is the aide doing this instead of the nurse?" Explain the team model honestly: the nurse directs and remains responsible for the plan of care; trained team members perform selected care under nurse supervision. Never imply the patient has been handed off entirely.

Safety Considerations

The delegatee has the right — and duty — to refuse a task beyond their training or comfort, and the delegating nurse must accept that refusal, not pressure past it. Delegation under pressure ("just this once") is the classic root cause in delegation-related sentinel events. Document what was delegated, to whom, the directions given, and the follow-up performed.

NCLEX Clinical Judgment Points (CJMM)

Recognize cues: stem words stable, routine, predictable → delegable context; first, new, unstable, changed, evaluate, teach → not delegable.
Analyze cues: identify which Right the question is testing — most wrong options violate exactly one Right.
Prioritize: if a delegation is already going wrong, the first action is protecting the patient (reassume the task), then addressing the delegatee.
Take action: choose options with specific, parameterized communication over vague ones.
Evaluate: the correct answer often includes the RN following up — options ending at handoff are usually incomplete.

Common NCLEX Traps

Choosing the option where communication sounds polite but lacks reporting parameters; assuming a task is delegable because UAP "usually" do it, ignoring the circumstance (post-op day 0, transfusion, abnormal last reading); treating delegation as finished once the task is assigned — the credited answer usually includes supervision or evaluation; confusing delegation (beyond the delegatee's traditional role) with assignment (within their role); and forgetting the delegatee shares accountability for accepting a task they weren't competent to perform.

Five NCLEX Clinical Pearls

The Five Rights are sequential gates — a task that passes "right task" can still die at "right circumstance." Always test the patient's current stability, not the diagnosis.
Specific reporting parameters ("call me if systolic <100") are the signature of right communication; vague vigilance requests are the signature of a wrong answer.
The credited delegation answer usually contains follow-up. If two options delegate correctly, the one including supervision/evaluation wins.
Delegate the collection of data, never its interpretation — UAP may measure the blood pressure; only the nurse decides what it means.
Accountability layers, never vanishes: the nurse answers for the decision and follow-up; the delegatee answers for accepting and performing; both can be liable in the same event.

Five Common Student Mistakes

Memorizing the five labels without being able to match a scenario error to the specific Right it violates — NCLEX tests the matching, not the list.
Treating "the patient is stable" as permanent — missing that a new abnormal finding revoked the circumstance mid-shift.
Believing written policies substitute for individual competency validation ("all UAPs here do accuchecks").
Thinking supervision means hovering — it means availability, defined checkpoints, obtaining results, and evaluating outcomes.
Assuming the delegatee bears no responsibility because "the RN is accountable" — the delegatee is always accountable for their own performance and for refusing tasks beyond their competence.

Five Application Scenarios

Right task/circumstance: The RN asks UAP to take vitals on four patients, including one who began a blood transfusion 10 minutes ago. Error: right task, wrong circumstance — the RN must take the transfusion patient's vitals herself during the first 15 minutes.
Right person: A newly hired UAP is asked to obtain a capillary blood glucose. She has not yet completed the facility's competency validation. Correct action: RN performs it or assigns a validated UAP — job description alone does not establish right person.
Right communication: The RN tells UAP, "Ambulate Mr. Diaz today." Better: "Ambulate Mr. Diaz 100 feet with his walker twice this shift; stop and report to me immediately if he becomes short of breath or dizzy." The second version passes the right — specific task, parameters, and reporting triggers.
Right supervision: UAP reports completing all morning vitals. The RN charts "vitals obtained." Missing step: the RN must actually review the values and evaluate them — supervision includes obtaining and interpreting results, and one reading of 88/50 is buried in that list.
Refusal: UAP is asked to apply a sequential compression device she's never used. She says she isn't comfortable. Correct RN response: perform or reassign the task and arrange training later — pressuring her past refusal violates right person and creates shared liability.
