Why This Disease Matters

Prevalence: Aortic stenosis most common valvular disease in older adults, affects 2-4% over 65, calcific. Mitral valve prolapse affects 2-3% population. Rheumatic mitral stenosis still prevalent globally. AAA affects 4-8% men >65, prevalence rises with smoking, 5th leading cause death in men >65 when ruptures. Aortic dissection incidence 3 per 100,000 but 40% die immediately, 1% per hour mortality if untreated.

Patient safety: Aortic stenosis triad syncope + angina + dyspnea = valve area <1cm² critical — syncope from low CO on exertion, angina from hypertrophied LV needs more O2 than coronary supply, dyspnea from HF → sudden death risk. AAA rupture → hemorrhagic shock death within minutes. Aortic dissection tears aortic wall → malperfusion stroke/paraplegia/renal failure, rupture tamponade → death if BP/HR not controlled immediately. Deep palpation of large AAA can rupture.

Pathophysiology

Normal: Aortic valve opens systole blood LV → aorta, closes diastole prevents backflow. Mitral valve opens diastole LA → LV, closes systole chordae prevent prolapse. Aorta elastic intima/media/adventitia handles pressure. ↓ Aortic Stenosis — What goes wrong: Calcific degeneration age/bicuspid valve/rheumatic → cusps fused calcified cannot open → fixed obstruction → LV must generate higher pressure to push through → concentric LVH (WHY: wall thickens to handle afterload) → ↑ O2 demand + ↓ coronary perfusion (coronaries fill diastole but high LV pressure compresses) → angina even without CAD. Fixed CO cannot ↑ with exertion → brain low perfusion syncope on exertion. LVH stiff → diastolic dysfunction → ↑ LA pressure → pulmonary congestion dyspnea. Valve area normal 3-4cm², severe <1cm² gradient >40mmHg. Valvular Disorders
