Why This Matters

Prevalence: liver disease is common and rising—viral hepatitis affects millions, and cirrhosis (from alcohol, hepatitis C, and fatty liver disease) is a leading cause of death. Its complications fill medical and ICU beds.

Patient safety: a failing liver loses its jobs—clotting, detoxifying, and making proteins—so patients face bleeding, confusion, and fluid overload. Two complications are true emergencies: esophageal variceal hemorrhage (massive bleeding) and hepatic encephalopathy (ammonia-induced coma). Recognizing them fast saves lives.

Analogy — a clogged sponge and a dammed river

A healthy liver is a soft sponge that blood flows through easily while it cleans and processes it. In cirrhosis the sponge turns to scar (hard and clogged)—so it can't process blood (→ toxins/ammonia build up → confusion) or make its products (→ bleeding, low albumin, jaundice). Blood trying to get through hits the clog like a dammed river (portal hypertension) and floods over the banks into side channels—swelling fragile veins in the esophagus (varices that can burst) and leaking fluid into the belly (ascites). One clog, a whole cascade.

Pathophysiology

Normal physiology: blood flows easily through the soft liver, which makes clotting factors/albumin, stores glucose, makes bile, and converts ammonia to urea and detoxifies drugs. ↓ What goes wrong: chronic injury (alcohol, hepatitis B/C, fatty liver) inflames and then scars (fibroses) the liver → cirrhosis. Scarring blocks blood flow (portal hypertension) and destroys functioning liver cells. ↓ How symptoms appear: Portal hypertension → blood backs up → esophageal varices (can rupture), ascites, and splenomegaly (→ low platelets). Lost function → bleeding (↓clotting factors, ↑INR), edema/ascites (↓albumin), jaundice (↑bilirubin), and confusion (↑ammonia → hepatic encephalopathy). ↓ How it progresses: compensated cirrhosis → decompensation with variceal hemorrhage, refractory ascites, spontaneous bacterial peritonitis, hepatorenal syndrome, encephalopathy/coma, and liver cancer. Treatment controls complications; a transplant may be needed. High-risk populations: people with chronic alcohol use, chronic hepatitis B/C, or nonalcoholic fatty liver disease/obesity/diabetes; IV drug users; and those with prior blood exposures. Alcohol and hepatitis C are leading cirrhosis causes—both are largely preventable/treatable. Vaccinate against hepatitis A and B (no vaccine for C).

Why a bleeding varix is a can't-wait emergency Esophageal varices are thin-walled, high-pressure veins. If one ruptures, bleeding is massive and rapid—the patient vomits large amounts of blood and can go into hypovolemic shock in minutes. Worse, cirrhosis impairs clotting (↑INR, ↓platelets), so the bleeding won't easily stop. This is why prevention (beta-blockers, banding) and rapid response (octreotide, endoscopy, fluids/blood) are critical.

Nursing Assessment

Subjective: alcohol/drug use, hepatitis history/exposures, fatigue, appetite/weight changes, abdominal swelling, bleeding/bruising, confusion (often reported by family), and medication use (hepatotoxins).

Objective: mental status/LOC (encephalopathy), jaundice/skin, abdomen (ascites, girth, splenomegaly), edema, signs of bleeding (bruising, emesis/stool), asterixis, and vital signs. Correlate with liver enzymes, ammonia, INR, albumin, and platelets.

Patient teaching — paracentesis & biopsy For paracentesis, explain it relieves the pressure of ascites and requires an empty bladder (to avoid puncture); expect improved breathing/comfort after. For biopsy, explain the need to lie on the right side afterward to compress the site and prevent bleeding (the liver is very vascular and clotting is impaired).

Why lactulose "diarrhea" is the goal, not a side effect Lactulose works by pulling ammonia into the gut and flushing it out in stool. So the 2–3 soft stools per day are the therapeutic goal—if the patient isn't stooling, the ammonia isn't leaving and confusion worsens. Nurses titrate the dose to stool output and never withhold it just because stools are loose. Watch potassium, since diarrhea can lower it.

Patient Education

Disease process: explain that the liver is scarred and can't do its jobs, causing bleeding, fluid buildup, and confusion—and that the goal is to prevent complications and protect remaining function. Lifestyle: completely avoid alcohol (the most important change), avoid hepatotoxic drugs (limit acetaminophen to safe doses, avoid NSAIDs and unnecessary sedatives), maintain nutrition with adequate calories and moderate protein and a low-sodium diet (for ascites), and manage weight/diabetes (fatty liver). Hepatitis prevention: get hepatitis A and B vaccines, practice safe sex and needle safety, and understand transmission (hep A/E fecal-oral; hep B/C/D blood/body fluids). Medication adherence: take lactulose as prescribed (aim for 2–3 soft stools/day—don't stop for loose stools), beta-blockers and diuretics as directed, and complete antiviral therapy. Bleeding precautions: soft toothbrush, electric razor, avoid straining/heavy lifting (varices), and report bleeding/bruising. Home care: daily weights and report rapid gain or increasing abdominal girth; monitor for confusion. Follow-up: keep hepatology, lab, endoscopy, and cancer-surveillance appointments. Clinical Judgment (NCSBN Model)

Recognize cues: a client with cirrhosis is admitted with increasing confusion, disorientation, and a flapping tremor (asterixis). Labs: ammonia elevated, INR 1.9, albumin 2.5, platelets 90,000. He recently became constipated and missed his lactulose.
Analyze cues: confusion + asterixis + high ammonia = hepatic encephalopathy, triggered by constipation/missed lactulose; the coagulopathy (↑INR, ↓platelets) adds bleeding risk.
Prioritize hypotheses: encephalopathy with safety risk is the priority—protect the confused patient and lower ammonia; watch for bleeding.

Case Study — from admission to discharge

Admission. Mr. Delgado, 58, with alcoholic cirrhosis, is brought to the ED vomiting a large amount of bright red blood. He is pale, tachycardic (HR 122), and hypotensive (BP 86/50). He also appears mildly confused. Labs: Hgb 7.8, INR 2.1, platelets 78,000, ammonia elevated.

Step 1–2: Recognize & analyze cues

Cues: hematemesis + hypotension/tachycardia + low Hgb = esophageal variceal hemorrhage (from portal hypertension), worsened by coagulopathy (↑INR, ↓platelets). Mild confusion + high ammonia suggests early encephalopathy. This is an emergency.

Step 3–4: Prioritize & plan

Priority: airway and circulation. Plan: protect the airway (aspiration risk), give oxygen, establish large-bore IV access, IV fluids and blood products, start octreotide, prepare for emergent endoscopy (band ligation), monitor VS/H&H, and maintain bleeding precautions. Address encephalopathy with lactulose once stabilized.

Step 5: Take action

The nurse positions to protect the airway, applies oxygen, gives fluids/blood and octreotide, monitors VS and mental status, and readies the patient for endoscopy. Bleeding is controlled with banding; lactulose is started for the elevated ammonia.

Step 6: Evaluate & discharge

VS stabilize, bleeding stops, and mental status clears with lactulose (2–3 stools/day).
